Alternatively, you can scan the QR code provided by your organization to be taken directly to the reporting page. If you’re on a mobile device, scanning the code will take you directly to your organization’s secure FaceUp portal.
-Once you enter your unique code or scan the QR code, you will be redirected to a secure page where you can submit your report anonymously.
